OCR description
The covered entity (CE), Dental Health Services, reported that it inadvertently emailed the protected health information of (PHI) of 1,778 individuals to the wrong recipients. The PHI involved included names, dates of birth, member identification numbers, and health insurance information. The CE notified HHS and the affected individuals. In its mitigation efforts, the CE promptly conducted an internal investigation and secured the affected systems. The CE also implemented additional administrative and technical safeguards. Staff were retrained to better protect PHI.
