OCR description
The business associate (BA), Philips Respironics, reported that a software application used by its business associate (BA) exposed the protected health information (PHI) of 5,576 individuals. The PHI involved included names, dates of birth, addresses, and treatment information. The BA notified HHS, the affected individuals, the media, and posted substitute notice on its website. In response to the breach, the BA provided complimentary credit monitoring services and implemented additional administrative and technical safeguards to better protect PHI.
