OCR description
The covered entity (CE), Montgomery County Board of Developmental Disabilities Services, reported that it experienced a hacking incident that affected the protected health information (PHI) of 8,819 individuals. The PHI involved included names, addresses, dates of birth, social security and drivers’ license numbers, financial and claims information, and diagnoses and conditions. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional technical and security safeguards. OCR provided technical assistance.
