OCR description
The covered entity (CE), Justice Resource Institute, reported that it experienced a ransomware attack that affected the protected health information (PHI) of 23,118 individuals. The PHI involved included names, social security numbers, addresses, birthdates, diagnoses/conditions, and medications. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE provided complimentary credit monitoring services and implemented additional physical, technical, and security safeguards to better protect its PHI.
