OCR description
The business associate (BA), Signature Healthcare Services LLC, reported that it experienced a ransomware incident that affected the protected health information (PHI) of 14,696 individuals. The PHI involved included names, dates of birth, social security numbers, medical record numbers, services received, dates of services, and treating physicians. The BA notified HHS, affected individuals, the media, and posted substitute. In response to the breach, the BA offered free credit monitoring to affected individuals and implemented additional administrative and technical safeguards.
