OCR description
The business associate (BA), Panoramic Health, reported that it experienced a hacking incident that affected the protected health information (PHI) of 5,445 individuals. The PHI involved included names, dates of birth, addresses, diagnoses, medications, and financial and claims information. The BA notified HHS, affected individuals, and the media. In response to the breach, the BA changed passwords and strengthened password requirements, implemented new technical safeguards, and retrained workforce members.
