OCR description
The covered entity (CE), Winter Haven Hospital, reported that a workforce member sent an email containing the protected health information (PHI) of 2,101 individuals to the wrong recipient. The PHI involved included names, dates of birth, email addresses, phone numbers, and dates of service. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach, the CE sanctioned the employee and provided retraining on email security.
