OCR description
The covered entity (CE), 1st Choice Home Care, reported that an employee shared the protected health information (PHI) of 1,600 individuals with others. The PHI involved demographic and clinical information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards.
