OCR description
The covered entity (CE), Texas Panhandle Centers, reported that it experienced a ransomware incident that affected the protected health information (PHI) of 16,394 individuals. The PHI involved included names, dates of birth, Social Security numbers, addresses, drivers’ license numbers, claims information, diagnoses and conditions, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards. OCR provided technical assistance regarding the HIPAA Rules.
