OCR description
The covered entity (CE), Badalamenti-Ferlito Orthodontics, reported that it experienced a breach incident that resulted in the disclosure of the protected health information of 1,427 individuals to an unauthorized party. The CE provided notice to HHS and the affected individuals. In response to the breach, the CE implemented additional administrative and technical safeguards, and retrained its workforce.
