OCR description
The covered entity (CE), Montefiore Einstein Advance Care, reported that it was the target of a cyberattack that compromised the protected health information (PHI) of 2,356 individuals. The PHI involved included names, addresses, dates of birth, social security numbers, medical record numbers, and dates of service. The Ce notified HHS, the affected individuals, and the media. In response to the breach, the CE implemented additional administrative and technical safeguards to better protect its PHI.
