iCircle Services of the Finger Lakes, Inc.

The covered entity (CE), iCircle Services of the Finger Lakes, reported that an employee was the subject of an email phishing incident that affected the protected health information (PHI) of 3,414 individuals. The PHI involved included names, addresses, phone numbers, email addresses, dates of birth, Social Security and drivers’ license numbers, passport numbers, financial and claims information, diagnoses, medications, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional administrative, technical, and security safeguards. Staff were retrained on email security.
