OCR description
The covered entity (CE), FC Compassus, reported that an employee was the target of an email phishing attack that compromised the protected health information (PHI) of 1,383 individuals. The PHI involved included names and treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional administrative and technical safeguards.
