OCR description
Mending Minds dba Walnut Creek Psychiatry, the covered entity (CE), reported that an employee impermissibly accessed the patient medical records of 733 individuals. The protected health information involved included names, addresses, birthdates, drivers’ license numbers, other identifiers, financial information, diagnoses/conditions, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE provided HIPAA training to its workforce and implemented new administrative, technical, and physical safeguards. OCR provided technical assistance regarding the HIPAA Rules.
