OCR description
The covered entity (CE), Panorama Eyecare, reported that an unauthorized individual obtained access to their network and exfiltrated data. The CE reported that the PHI included demographic, financial and clinical information for 377,911 individuals. Following the discovery of the incident, the CE provided notice to HHS, the affected individuals, and the media. In response to the breach, the CE updated security and technical safeguards. Staff also received anti-phishing training across the entire organization.
