OCR description
The covered entity (CE), Warehouse Employees Local 169 Contributing Employers Health and Welfare Fund, reported that its business associate (BA) experienced a ransomware attack that compromised the protected health information (PHI) of 1,061 individuals. The PHI involved included names, social security numbers, phone numbers, and account usernames. The BA notified HHS, affected individuals, and the media.
