OCR description
The covered entity (CE), IBEW LOCAL 236 WELFARE FUND, reported that its business associate (BA) experienced a cybersecurity incident that affected the protected health information (PHI) of 3,217 individuals. The PHI included names, social security numbers, phone numbers, and claims information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE and BA implemented additional administrative, technical, and security safeguards to better protect PHI.
