OCR description
Consulting Radiologists, the covered entity and business associate (CE/BA), reported that it experienced a ransomware intrusion affecting the protected health information (PHI) of 583,824 individuals. The PHI involved included names, dates of birth, social security and drivers’ license numbers, health insurance information, diagnostic codes, imaging reports, and other treatment information. The CE/BA notified HHS, the affected individuals, and the media. In response to the breach, the CE/BA provided complimentary credit monitoring services and implemented additional administrative, technical, and security safeguards.
