OCR description
The covered entity (CE), Radiology and Imaging Specialists, reported that an employee was the target of an email phishing scheme that affected the protected health information (PHI) of 37,210 patients. The PHI involved included clinical and demographic information. In response to the breach, the CE implemented additional administrative, technical, and security safeguards, and retrained workforce members to better protect its PHI.
