OCR description
The business associate (BA), Kairos Health Arizona, Inc., reported that a former BA accessed its database that contained the protected health information (PHI) of 14,435 individuals. The PHI involved included dates of birth, claims information, diagnoses/conditions, and other treatment information. The BA notified HHS, affected individuals, and the media. In response to the breach the BA implemented additional technical and administrative safeguards to better protect PHI.
