OCR description
The covered entity (CE), Providence Mission Heritage Endocrinology, reported that an employee impermissibly accessed the protected health information (PHI) of 1,875 individuals. The PHI involved included state identification and drivers’ license numbers and health insurance information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE provided free credit monitoring services and implemented additional administrative and technical and security safeguards.
