OCR description
The covered entity (CE), Core Dental Health, reported that it was the target of a ransomware attack that compromised the protected health information (PHI) of 2,349 individuals. The PHI involved names, dates of birth, addresses, and clinical information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative and technical safeguards.
