OCR description
The business associate (BA), Johnson & Associates, reported that it experienced a cyber-attack that compromised the protected health information (PHI) of 4,600 individuals. The PHI involved included names, addresses, and birthdates. The BA notified HHS, affected individuals, and the media. In response to the breach, the CE terminated its business relationship with the BA.
