OCR description
The North Cottage Program, the covered entity (CE), reported that it experienced a hacking incident that affected the protected health information (PHI) of 8,190 individuals. The PHI involved included names, date of birth, addresses, drivers’ license and Social Security numbers, clinical diagnoses, lab results, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative and technical safeguards. OCR provided technical assistance regarding the HIPAA Rules.
