OCR description
The covered entity (CE), Barrett Eye Care, reported that it experienced a ransomware incident that affected the protected health information (PHI) of 4,648individuals. The PHI involved demographic and financial information. The CE notified HHS, affected individuals, the media, and posted substitute notice in a major print publication. In response to the breach, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
