OCR description
NeighborHealth, the covered entity (CE), reported that, due to an error on its part, its business associate (BA) mailed the protected health information (PHI) of 526 individuals to the wrong recipients. The PHI involved were names only. The CE notified HHS, the media and posted substitute notice on its website. In its mitigation efforts, the CE retrained its staff and implemented additional administrative safeguards.
