OCR description
The covered entity (CE), Special Health Resources of Texas Inc., reported that it experienced a ransomware incident that affected the electronic protected health information (ePHI) of 13,071 individuals. The ePHI involved included names, addresses, dates of birth, social security numbers, Medicaid/Medicare numbers, medical conditions, and other treatment information. The CE notified HHS, the affected individuals and the media. In response to the breach, the CE implemented additional system monitoring safeguards and additional technical safeguards.
