OCR description
The covered entity (CE), Internal Medicine Associates dba Gastrointestinal Medicine Associates, reported that it was the subject of a ransomware attack that affected the protected health information (PHI) of 31,853 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented multi-factor authentication across all users, retrained workforce members, and implemented new technical safeguards.
