PDG, P.A. dba Park Dental

The covered entity (CE), PDG dba Park Dental, reported that several of its employees were the targets of an email phishing scheme that affected the protected health information (PHI) of approximately 185,606 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license numbers, social security numbers, financial information, diagnoses/conditions, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE provided security awareness training to its workforce and enhanced its access controls to better protect its PHI. OCR provided technical assistance regarding the HIPAA Rules.
