OCR description
The covered entity (CE), Fairfield Memorial Hospital Association, reported that it experienced a ransomware attack that compromised the protected health information (PHI) of 25,093 individuals. The PHI involved included names, dates of birth, social security numbers, health insurance information, diagnoses, conditions, and medical treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
