OCR description
Behavioral Health Alliance of Rural Pennsylvania, the covered entity (CE) reported that an employee was the subject of an email phishing scheme that compromised the protected health information (PHI) of 642 individuals. The PHI involved included names and treatment information. The CE notified HHS, affected individuals, and the media. In response to the incident, the CE strengthened its administrative and technical safeguards and retrained its staff on email security.
