OCR description
The covered entity (CE), HAH Group Holding Company dba Help At Home, reported that its vendor experienced a cyberattack that affected the protected health information (PHI) of 26,744 patients. The PHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses, conditions, claims and financial information, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE terminated its business relationship with the vendor.
