OCR description
The covered entity (CE), Mid-Columbia Center for Living, reported that it experienced a phishing incident that affected the protected health information (PHI) of 4,435 individuals. The PHI involved included patient names, address, ID number, medical record number, Social Security number, treating physician, treatment information, treatment dates, diagnoses/conditions, Medicaid number, and health insurance information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE offered free credit monitoring and implemented additional administrative, technical, and security safeguards.
