OCR description
The covered entity (CE), Connecticut GI, reported that it experienced a cyber-attack that compromised the protected health information (PHI) of 46,354 individuals. The PHI affected included names, dates of birth, addresses, Social Security and drivers’ license numbers, financial information, and treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE offered complimentary credit monitoring and identity protection services and implemented additional administrative and technical safeguards.
