The covered entity (CE), Advanced Medical Management, reported that several employees were the targets of an email phishing scheme that compromised the protected health information (PHI) of approximately 2,815 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license and social security numbers, claims information, diagnoses/conditions, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE enforced multi-factor authentication, increased its workforce security awareness training, and increased its threat protections to better protect its PHI.
