OCR description
Compass Healthcare & Rehab Rowan, the covered entity (CE), learned that employees were the target of an email phishing incident that affected the protected health information (PHI) of 1,381 individuals. The PHI included names, dates of birth, addresses, drivers’ license and Social Security numbers, financial information, diagnoses and conditions, lab results, and other treatment information. The CE notified HHS, affected individuals, and the media. The CE implemented additional administrative and technical safeguards. OCR provided the CE with technical assistance regarding the HIPAA Rules.
