OCR description
The covered entity (CE), Catholic Charities CYO of The Archdiocese of San Francisco, reported that it experienced a cyberattack that compromised the protected health information (PHI) of 7,486 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license and Social Security numbers, financial information, diagnoses and conditions, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach the CE implemented additional technical safeguards to better protect its PHI.
