OCR description
The covered entity (CE), Oficina Medica, reported that three computers, one printer, one telephone, and several scanners were stolen from its clinic. The protected health information (PHI) of 2,618 individuals was affected. The PHI involved included names, addresses, dates of birth, diagnoses/conditions, lab results, and medications. The CE notified HHS and the affected individuals. In response to the breach, the CE implemented additional administrative, technical, and security safeguards.
