OCR description
The covered entity (CE), La Liga Puertorriquena Contra El Cancer dba Hospital Oncológico, reported that an employee was the subject of an email phishing incident that affected the protected health information (PHI) of 4,590 individuals. The PHI involved included names, addresses, Social Security and drivers’ license numbers, diagnoses, and birthdates. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
