OCR description
The business associate (BA), J. Taylor & Associates, LLC, reported that an employee was the subject of an email phishing scheme that affected the protected health information (PHI) of 22,316 individuals. The PHI involved clinical, demographic, and financial information. The BA notified HHS, the affected covered entities, the affected individuals, and the media. In response to the breach, the BA reset passwords, implemented additional technical safeguards, and conducted additional security awareness training.
