OCR description
Jefferson PA Heart and Vascular, the covered entity (CE), reported that an employee mailed the protected health information of 755 individuals to the wrong recipients. The PHI involved included names and treatment information. The CE notified HHS and the affected individuals. In response to the breach, the CE strengthened its administrative safeguards.
