OCR description
The covered entity (CE), UT Southwestern Medical Center, reported that an unauthorized individual gained access to the protected health information (PHI) of 778 individuals by impersonating a workforce member. The PHI involved included names, addresses, dates of birth, social security numbers, financial and claims information, diagnoses, conditions, and other treatment information. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE sanctioned the workforce member involved and revised its hiring process.
