OCR description
The covered entity (CE) Holistix Treatment Center dba We Level Up Treatment Lake Worth reported that it experienced a hacking attack that affected the protected health information (PHI) of 6,567 individuals. The PHI involved included names, mailing addresses, and Social Security numbers. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach, the CE implemented additional administrative and technical safeguards.
