OCR description
The covered entity (CE), Community Clinic of Maui dba Malama I Ke Ola Health Center, reported that it experienced a cybersecurity incident that affected the protected health information (PHI) of 123,816 individuals. The PHI involved included financial, clinical and demographic information. The CE notified HHS, the affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
