OCR description
The covered entity (CE), Hafetz and Associates, reported that multiple employees were the subjects of an email phishing scheme that affected the protected health information (PHI) of 26,474 individuals. The PHI involved included names, dates of birth, Social Security and drivers’ license numbers, addresses medications, claims and financial information, and health insurance information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
