OCR description
The covered entity (CE), Tri-County Medical Supply & Respiratory Services, reported that a former employee improperly disposed of the protected health information (PHI) of 8,000 individuals. The PHI involved included names, dates of birth, Social Security numbers, drivers’ license numbers, addresses, diagnoses/conditions, and other treatment information. In response to the breach, the CE provided complimentary credit monitoring services to the affected individuals and retrained workforce members to better protect its PHI.
