OCR description
The covered entity (CE), Sarah Bush Lincoln Fayette County Hospital, reported that an employee was the target of an email phishing scheme that compromised the protected health information (PHI) for 4,845 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license and Social Security numbers, diagnoses/conditions, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative and technical safeguards. OCR provided technical assistance regarding the HIPAA Privacy Rule.
