OCR description
The covered entity (CE), Chicago Cardiology Institute, reported that an employee was the target of an email phishing scheme that affected the protected health information (PHI) of approximately 2,743 individuals. The PHI involved included names, dates of birth, and treatment information. The CE notified HHS, the affected individuals, and the media. In its mitigation efforts, the CE provided complimentary credit monitoring services and strengthened its technical safeguards.
