OCR description
The covered entity (CE), Lejla Streets, D.M.D dba Smiles on the Pines, reported that her former landlord gained access to her office when she was not present and the protected health information (PHI) of 1,800 individuals was potentially compromised. The CE notified HHS. After filing the breach report, the CE determined that no PHI was breached. In addition, the entity is no longer in business.
