GPS Sango Family Dentistry, PLLC d/b/a Sango Family Dentistry

GPS Sango Family Dentistry dba Sango Family Dentistry, the covered entity (CE), reported that it experienced a hacking incident that affected the protected health information (PHI) of 27,000 individuals. The PHI involved included names, dates of birth, addresses, telephone numbers, email addresses, Social Security numbers, health insurance information, diagnoses and conditions, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional technical safeguards to better protect sensitive data.
